School Admissions - Accept or Decline the Offer of a School Place (Infant, Junior, Primary)

Only use this form to either accept or decline the offer of a school place for your child if your child is due to start reception class, or move from an Infant to a Junior or Primary school in September, as indicated in your offer email or letter.

This form must be submitted by 4.30pm on Tuesday 23 April 2024. at the latest. Failure to meet this deadline could seriously jeopardise the chances of you securing a place for your child at your preferred school. If you have any queries, please contact the School Admissions Team on 0191 424 7767 / 424 7706. Please note that in order to accept a school place, you must have received an offer of a school place via your e-mail account or by letter. All acceptances that are submitted are checked and verified by the School Admissions Team.

If you accept a school place that your child has not been offered, The School Admissions Team will contact you to confirm that this will not be processed.

Steps to complete this form

  1. Please read the Notes page carefully before completing the form
  2. Complete your child's details carefully and the School place offered
  3. Either accept the place or decline supplying a reason
  4. Complete the declaration page
  5. Make a note of your Transaction Reference Number
