Steps to complete this form

1. Enter Applicant or Agent Details.
2. Enter Address of Building to be Demolished.
3. Enter Contact/Keyholder Details.
4. Enter Further Details (timescale/extent of demolition).

Items you will need to complete this form

1. Names and Addresses of Agent, Contact and Keyholder.
2. Extent of the property(s) to be demolished and the presence of utilites and hazardous materials located within.
3. Proposed demolition date and timespan to complete demolition.
